"where should I be recording evidence of completion of competencies ?"
Trainees should log all the evidence in the portfolio tool provided by their parent specialty. When they are in a post in another specialty they can use the forms designed specifically for that specialty, but again these should be recorded within their parent specialities portfolio. It is not necessary to use a separate portfolio to log assessments in each of the different modules of ACCS training. For further information see the 'Portfolio' section of this website.
